At our practice, we believe that mental health is an essential part of a child’s overall well-being. We offer thoughtful, evidence-based care for children and adolescents facing challenges such as ADHD, anxiety, and depression. Our approach emphasizes strong partnerships—with families, schools, and local community resources—to create individualized strategies that support your child’s growth both at home and in daily life.

While we often begin with behavioral and environmental supports, medication may be considered as an adjunct therapy when appropriate. Our goal is to provide comprehensive, compassionate care tailored to your child’s unique needs.

To ensure a complete and accurate evaluation, we ask that families complete the required paperwork/surveys in advance of their appointment. This allows us to better understand your child’s experiences and provide the most effective care from the start.

ADHD:

  • Initial visit for diagnosis, Vanderbilt Assessment Scale forms completed by parents/guardians and by teachers are used to help with the diagnosis of ADHD. 
    • Parent/guardian- parent versions of the form will be sent via Phreesia to be completed electronically. For your convenience, below is a printable link that can be used if preferred over electronic forms
    • Teachers- at least three forms completed by different teachers are required for diagnosis. For your convenience, below is a printable link that can be given to your child's teachers.
