      • When you update your PCP, please ask the representative to retro the PCP to start of insurance, Date of Birth, or date of 1st visit to practice depending on your circumstances. They only allow you to retro 3 months prior to the date of change.
      • In order to change your PCP, you will be required to fill out a PCP change form in our office and CALL member services listed on your insurance card also. Once you contact your MCO for PCP change it could take 7-10 business days to update their system. 
      • If your PCP is not listed as one of our Physicians or Facility, we will not be able to schedule any visits until changed. 
    • Visits can only be schedules once we can confirm/verify that you have enrolled with an MCO we participate with and your PCP has been updated.
